Sick Visits (some sick visits may be through telehealth or with an outdoor/in car screening due to our covid precautions)

Important Information About Our Office
Due to the increased demand for healthcare services in our region, we are currently accepting new primary care patients on a selective basis.
We kindly request that you complete a prospective patient packet for our review. Upon submission, our office will reach out to you to discuss the next steps (Please be aware that completing the prospective patient packet does not automatically establish you as a patient of our clinic. This process is the first step in determining eligibility and fit for our services based on our current capacity and your specific healthcare needs)
For individuals who have been accepted to join the Jennas Promise Program and are opting to have medical services at our office, please fill out the same prospective patient packet. Upon review, we will coordinate with you to schedule your appointment.
